Appointment

Please arrive 15 minutes prior to your appointment to complete any necessary insurance paperwork.

• Please complete the new patient intake document to avoid delays in service

Please bring:

• Drivers license or legal photo ID
• Insurance card
• Any referrals if needed. It is advised that you call your insurance company ahead of time to see if a referral is needed
• All pertinent medical records

Payment Policy

Co-payments will be collected at each visit.

Insurance

We participate in all major health plans. Please contact our office for verification of your insurance.

No Insurance

Patient without insurance are welcome. Cost of the visit can be discussed prior to the service. We can offer payment plans upon request.

No Show/Cancellation Policy

Please let us know as soon as possible if you need to cancel or reschedule your appointment. We reserve the right to charge a fee for no-shows and/or cancellation with less than 24 hours of notice.
